Living Waters Family Medical Practice is committed to protecting your personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

What we collect

To provide safe, continuous medical care we collect personal and health information including your name, date of birth, contact details, Medicare and concession card numbers, next-of-kin, medical history, medications, allergies, immunisations, test results, and notes from consultations.

We also collect basic technical information when you use this website (pages viewed, device type, anonymised IP) via Google Analytics 4 to understand and improve the service.

How we use it

Information is used to provide medical care, communicate with you about appointments and results, claim Medicare/DVA rebates, meet our legal and professional obligations, and improve the quality of our service.

Who we share it with

We share information only when necessary for your care or when required by law. This may include other treating practitioners (specialists, hospitals, pathology, radiology), Medicare/DVA, the Australian Immunisation Register, and our online booking provider HotDoc for the limited purpose of managing your booking.

We never sell your information.

How we store and secure it

Records are stored in secure, password-protected clinical software. Paper records (where they exist) are kept in locked storage. Staff access is restricted on a need-to-know basis. Information is retained for the minimum period required by law (generally 7 years after the last consultation, or until age 25 for minors — whichever is longer).

Access and correction

You may request access to your health record or ask us to correct information you believe is inaccurate. Please put your request in writing to our Practice Manager. We may charge a reasonable fee to cover the cost of providing copies.

Cookies and analytics

This website uses Google Analytics 4 to measure traffic and improve usability. Analytics cookies do not identify you personally. You can opt out via your browser settings or Google's opt-out add-on. We do not run advertising or remarketing tags.
